Affidavit : The Georgia Security and Immigration Compliance Act (GSICA) requires that “every agency or political subdivision shall verify the lawful presence in the United States of any applicant for public benefits.” (O.C.G.A. § 50-36-1). Therefore, you will be required to complete an affidavit prior to receiving any service.

Authorized Representative Form : An authorized representative is a person or entity that has the legal authority to act on behalf of another person or entity. This form should be completed when an individual wants to allow another individual to act on their behalf as it relates to their Vocational Rehabilitation Services are concerned.
- U.S. Citizenship and Immigration Services (USCIS) I-9 in order to determine lawful presence: The USCIS I-9 is a form that verifies a new employee's identity and employment eligibility. GVRA must determine an individual’s lawful presence in The US for employment and can accept documents that are outlined on this form.
- SAO Vendor Management: The documents found at this link must be completed by GVRA clients that need to become vendors in order to receive reimbursement from GVRA.
